Medicaid/CHIP/Snap Application  

(if you already have a case, you will need updated income, proof of utilities and proof of rent/mortgage payments)

1. Proof of citizenship for all household members (Birth Certificates/Passports)
2. Driver’s license for primary applicant
3. If married and your ID  is in your maiden name, you will need a copy of your marriage certificate
4. Copy of all Social Security cards for all household members
5. Proof of Residency- Can be: Utility bills (should give in anyways for utility deduction for Food Stamps) or copy of Mortgage statement/Lease agreement (should give in also for shelter deduction for Food Stamps) or Bank statement or Letter stating residency in Cuyahoga County.
6. Income for 30 days
   – If just started a job a letter from employer with income/how often received
   – If self-employed-books or bookkeeper tel. # or tax return.
7. If pregnant, proof of pregnancy.
8. Benefits termination letter from previous County
